| Here is what we will
need to get started on your comprehensive energy review. If you need assistance obtaining this

Information Form,
and we'll
help you. Usage History Completion of the Meter History Letter of Authorization. This will authorize your TDSP to give us 12 months of meter readings. All you need to do is print your name and sign it in the section under "Authorization". We will complete the rest. |

Details If we are going to see if a mid-contract switch makes sense for you, we will need a recent bill. If your contract is coming due for renewal, we will need your renewal offer your current provider will send you about 30 days prior to contract end. We will help you if you don't have this available. |
